In order to qualify for this FREE program, individuals must also have a dual diagnosis of a Severe Mental Illness (SMI) or Severe Emotional Disorder (SED) diagnosis such as a Mood Disorder or Depression. We offer Peer Support Services for up to 3 hours per week provided by an assigned Peer Support Specialist for 90 days. Our Peer Support Services are structured, scheduled interventions offered in the community. We address the person’s identified needs and goals and assist with connections to Long-Term Care Services.
